ABSTRACT

Clinically significant thromboembolic disease originating from the paralyzed legs of hemiplegic patients can occur unexpectedly and may affect morbidity and mortality of such patients. Doppler and Duplex sonography, are simple noninvasive techniques, that can accurately reveal deep venous thrombosis of the lower limbs. Doppler and Duplex examination were performed on the lower extremities of 40 hemiplegic patients suffering from cerebrovascular stroke. The study showed that 25% of patients [10 of 40 patients] had evidence of deep leg vein thrombosis. 9 cases were detected in the paralyzed limbs, forming an incidence of 22.5%. Six were acute [15%] abd 3 chronic [7.5%]. One case was detected in the non-paretic limb, with an incidence of 2.5%. Patients in the age group of 60-70 years showed the highest incidence of phlebothrombosis, and increased incidence was also shown in patients with poor or no muscle power of the affected limb. It was concluded that Doppler and Duplex imaging can be successfully used in the early detection of DVT in high risk hemiplegic patient, thus leading to prompt medical management, and may reduce morbidity and mortality

ABSTRACT

Artificial insemination by husband's semen [AIH] was done for 36 females with variable causes of infertility [male factor, cervical factor and unexplained infertility] with at least one patient healthy tube. 101 trials were done during the period of the study. Transuterine intratubal insemination was done using intrauterine foley catheter. Success of the procedure was first tested during diagnostic laparoscopy for infertility and vaginal ultrasonography [US]. Assessment of true trials was done by vaginal US alone. In all the trials, the treated semen was successfully placed into the patent fallopian tubes. This easy, rapid and convenient technique should be the first line of choice when AIH is indicated

ABSTRACT

Seventy high-risk pregnant patients were followed up in the antenatal care outpatient clinic and in the obstetric department in Kasr El Eini hospital from January 1990 till March 1992. Biophysical profile testing and umbilical artery Doppler study were done during follow up for case and reported accordingly. Obstetric decisions were not affected by the results of the Doppler study but by the sum of whole examination, follow up and special investigations done. The prenatal outcome was matched with the results of biophysical profile and Doppler study. The predictive values for abnormal and normal results of the tested parameters were compared [72% and 80% with biophysical profile and 80% and 86% with Doppler study]. The results with Doppler study were higher, yet they were not that much statistically significant. The predictive values when both parameters were abnormal or normal together in the same case were much superior than with single parameters. Doppler umbilical artery flow studies are an important adjunct to the biophysical profile in high risk pregnancy. The final Obstetric decision should be based on both parameters and not on abnormal parameters

ABSTRACT

The suction irrigation tubes of 5 mm or less in diameter are currently used in all centers of operative laparoscpy allover the world. Time loss and interruption of the operative procedure is commonly fasced when there are thick materials to be removed. A new wider tube for suction and irrigation was designed by Hesham Murad and manufactured by Casmed of the United Kingdom and was used with 12 cases of operative pelviscopy both elective cases and acute emergenicy. The function of both tubes were compared together in every case. Time saving, better organ orientation without interruption of the operative procedure were noticed in all cases once the wide tubs was inserted and started its action. This wide suction tube is now successfully used with operative pelviscopy saving time, expenses and physical effort

ABSTRACT

The Gynocheck endometrial tissue sampler was used to obtain endometrial tissue from 80 consecutive patients presenting with infertility premenopausal bleeding and postmenopausal bleeding, without anesthesia. Then the classic cervical dilation and endometrial curetting [D. and C.] was done under general anesthesia. Patient's acceptances, thime taken adequacy of sample, and reliability for histopathological diagnosis were evaluated. Different pathological results were obtained and were correlated 100% with the samples taken by D. and C. The failure rate and inadequacy of sample was very limited to only one case [1.25%]. This method was found to be raid, easy, accepted by the patients and reliable. Also it is superior to the cytology samplers being more reliable
